digitaal worden ingevuld, niet op papier. <strong>Dit onderdeel is niet verplicht. Indien u geen vragenlijsten wenst te ontvangen kunt u nog steeds meedoen aan de studie.<\/strong><\/p>\n<hr \/>\n<p>Een link naar deze vragenlijst wordt u per e-mail verstuurd. Hiervoor moet u wel eerst toestemming hebben gegeven. De vragen betreffen uw ervaringen met de cyste controles en de invloed ervan op uw dagelijks leven. De vragenlijst kan persoonlijke vragen bevatten. Wij verwachten dat het invullen 5 tot 15 minuten in beslag zal nemen.<\/p>\n<hr \/>\n<p>Als u deelneemt aan de pati\u00ebntenvragenlijst zult u per e-mail een link ontvangen om de vragenlijst online in te vullen. U ontvangt de vragenlijst voor en na uw eerste en tweede controle en daarna steeds na elk controle moment tijdens de eerste drie jaar van de follow-up. De eerste vragenlijst bevat ook enkele extra vragen over uw achtergrond en (medische) voorgeschiedenis.
